In India, many health insurers hand day-to-day claims servicing to a TPA. The insurer underwrites the risk and pays the money; the TPA does the paperwork in between. A typical TPA:
TPAs are registered and supervised by IRDAI under its TPA (Health Services) regulations. Some insurers also run in-house claims teams and use no TPA for some products, so the policy document or e-card tells you who services your claim.
| Question | TPA | Insurer |
|---|---|---|
| Who carries the risk? | No | Yes |
| Who processes documents? | Often, under contract | Directly, if no TPA is used |
| Who decides and pays? | Recommends | Approves and pays |
| Who handles grievances? | First point of contact for service | Responsible to the policyholder |
A common confusion is to treat a TPA rejection as final. The insurer owns the decision and the grievance process, so complaints go to the insurer's grievance cell and, if needed, the Insurance Ombudsman.
Illustrative example. A policyholder is admitted for a planned surgery at a network hospital. The hospital's insurance desk sends a pre-authorisation request with the doctor's notes to the TPA. The TPA checks cover, waiting periods and sub-limits, approves an initial amount and informs the hospital. At discharge, the hospital sends the final bill; the TPA reviews it, the insurer approves, and the patient pays only the items the policy excludes, such as some consumables.
Where it slows down is the handover: a missing report, a second document request, a status the patient cannot see. Insurers and TPAs use insurance claims management software to read documents on arrival, apply the same triage rules each time and push each status change to the customer. For why the second request causes the most delay, see claims stall on the second document request.

TPA stands for Third Party Administrator. It is a company registered with IRDAI that services health insurance claims on behalf of insurers, including cashless approvals at hospitals and reimbursement claim processing.
The TPA processes the claim and makes a recommendation, but the insurer carries the risk and is responsible for the final decision and payment. Complaints should go to the insurer's grievance cell.
Your policy schedule or health e-card names the TPA, if one is used. You can also ask the insurer, and IRDAI publishes a list of registered TPAs on its website.
