Human-in-the-loop means an AI system prepares or recommends and a person reviews or approves before it counts. Where to place the human, and common mistakes.

| Role of the person | Approves each item before it takes effect | Watches and can step in | None in the moment |

| Suits | Payouts, credit, public posts | High-volume, low-risk routing | Reversible, rule-bound steps |
Most real systems mix all three: automate the routine, monitor the middle, and keep a person on the decisions that are costly to reverse.
In claims, AI is good at the reading and sorting: structuring photos, bills and discharge summaries, checking them against policy cover, and proposing a triage lane. People are needed where judgement and accountability live: approving the payout, handling suspected fraud, and talking to an upset customer. Claims triage software for insurers such as AutoClaims is built this way: rules propose fast track, standard or investigate with the reason, adjusters review triage decisions, and payouts are approved by people under maker-checker. The reasoning behind the two-person rule is set out in why every claim payout needs two people.
A motor claim arrives on WhatsApp with four photos and a repair estimate. The system reads the estimate, confirms the policy is active and the damage is covered, and suggests "fast track" because the amount is under the insurer's threshold and nothing looks unusual. An adjuster spends one minute on the summary, agrees, and a second person approves the payout. Another claim with a repair bill dated before the policy start is flagged "investigate" with that reason shown, and goes to an investigator. Amounts and thresholds are set by the insurer; this example is illustrative.

It means a person reviews, corrects or approves an AI system's output at defined points before it takes effect, so the AI prepares the work and an accountable human makes or confirms the decision.
In human-in-the-loop, a person approves each decision before it happens. In human-on-the-loop, the system acts on its own while a person monitors and can intervene or stop it.
Decisions on payouts, credit and fraud affect customers' money and rights and are hard to reverse. A human reviewer adds judgement, catches errors and remains accountable to the regulator and the customer.
